Creating a Form

The form-creation process consists of the following stages: analysis, design, and printing.

At the analysis stage, the designer must do the following:

  1. Analyze the contents of the form and decide on the names of the fields and the exact wording of the explanations.
  2. Select the most appropriate type of form (black and white form with raster borders, black and white linear form, color dropout form, etc.) and printing method (at a printing shop, on a printer, using a Xerox machine, published on the Internet, etc.).
  3. Make sure that the form is machine-readable.

At the design stage, the designer must do the following:

  1. Create a form on the computer.
  2. Publish the form, i.e. lock it for editing so that no changes can be made to the form design (this will ensure that one and the same source form is used for printing all forms which are exactly the same).

At the distribution stage, the form is

  1. prepared for distribution
  2. distributed in hard copy or electronically
